Alzheimer's Association Connecticut Chapter: Needs Assessment Survey

The Alzheimer’s Association has been awarded funding from a Building Our Largest Dementia (BOLD) Infrastructure for Alzheimer's Act grant through the Connecticut Department of Public Health to conduct Alzheimer’s Disease and Related Dementias (ADRD) educational training for both medical and non-medical professional target groups.
In order to identify ADRD-related knowledge gaps and education and training needs throughout the State of Connecticut, we have developed a needs assessment that we hope you will participate in.
Your responses will be kept confidential and will only be used for research purposes. This survey is anonymous and will take approximately 5 minutes to complete.
