Reframing the Conversation of Social Determinants of Health

Reframe the link between homelessness and social determinants of health using a person-centered, strengths-based, and trauma responsive approach to care. The primary goal of this training is to introduce the term SDOH, formalize the term, while also reinforcing those with existing skills and competencies so that RSCs become more productive and resourceful. This training will inform the development of new programs and partnerships that ultimately improve health outcomes, reduce recidivism, and health care spending. Making the social determinants of health a core component of all RSC’s learning pathways is an essential tool and skill, and is integral to their education and training. As they progress through their educational programs and their careers, RSCs can gain greater understanding of the social determinants of health, how to partner within and outside of the health sector, and with communities through formal and informal continuing professional development.
Learning Objectives:
1.Introduce the term Social Determinants of Health.
2.Increase provider awareness of the life circumstances, the clinical and emotional conditions, and the other factors affecting our patients/members who are experiencing homelessness.
3.Identify how to properly use resources for populations experiencing homelessness.
